Registered Nurses, Care Managers, Intake Coordinators and Care Advisors - These positions work collaboratively as a case management team to assess clients, establish and implement a Plan of Care, conduct regularly scheduled home visits, and oversee client care. The team first ensures that clients meet eligibility requirements, that caregivers receive the appropriate credentials, and that their homes meet state requirements.  The team then provides ongoing support and training to caregivers and clients.


General Qualifications: Minimum of two years’ experience in case management, care planning and assessment for elders/disabled adults in a health care, long-term care, social service or community setting.  Strong clinical, communication and technology skills are essential. Regular car travel within an assigned regional geography is required for field positions.


Registered Nurses:  Must be fully licensed in the state in which they will practice and have recent experience in the direct care of the elderly and/or disabled. 


Care Managers and Intake Coordinators:  Bachelor’s degree required and current social work license, or an equivalent combination of education and experience in care of the elderly and/or disabled adults or related fields. Knowledge of case management systems, community service and delivery systems for aging and disabled population required. 


Care Advisors:  Bachelor’s degree preferred (or associate’s degree and equivalent elder care experience) as well as at least two years’ experience with provider network systems in human services or health care for elderly and/or disabled adults.  Call Center Experience extremely helpful.
