Caregiver Homes
Most seniors prefer to live in the community as they age. But for
elders who need 24-hour care or supervision, the right to choose care
at home often is limited by cost or access to care.
Seniorlink’s Caregiver Homes program creates a choice for these elders when confronted with the need for long-term care.
A Caregiver Home is the actual home of a caregiver who is paid to take
in and care for a senior who requires daily assistance with personal
care, as well as extensive support for meal preparation, transportation
and other activities that enable him or her to remain an active member
of the community.
Caregiver Homes is an innovative model of providing care for elders in
a home environment. Known as “individualized assisted living,” it is
unique from other long-term care options:
- Innovative technology connects each home with other members of the resident’s care team -- including nurses, case managers, physicians and even family members.
- Caregivers may be relatives.
- Each Caregiver Home is subject to two levels of
clinical oversight, including frequent on-site visits by the resident’s
case manager.
- There is only one resident per home, allowing for individual attention.
Caregiver Homes is now providing services as part of the Enhanced Adult Foster Care Program sponsored by the Massachusetts Office of Elder Affairs and Office of Medicaid.
