delivered by a nationwide network of professional geriatric care

Importantly, Seniorlink does not provide direct services itself and
maintains no affiliation with individual service providers. Families
always receive objective advice and referrals to the best quality
providers.
In-Home Assessments
An In-Home Assessment identifies the support required to help a senior
to live safely—either at home or in another setting. During a visit to
the elder’s home, a care manager evaluates 20 areas of critical
functioning, like personal care, mobility and cognition.
An In-Home Assessment is useful for families and long-distance
caregivers who are uncertain how to help a senior. It can assist family
members in reaching a consensus, including the decision to care for an
elder at home or in an assisted living or nursing home facility.
Care Management Consultations
A Care Management Consultation provides a link to the best service
providers in the community. During a visit to the elder’s home, a
Seniorlink Care Manager advises the family about specific services and
service providers available to help carry out a caregiving plan. This
consultation is best for families that do not require a full needs
assessment.
Ongoing Care Management
Seniorlink can develop a detailed and customized plan of care to
support a senior’s needs, drawing on locally-available resources for
transportation, meal preparation, home healthcare and other services. A
Seniorlink care manager can remain involved in the eldercare plan by
monitoring services and the senior’s changing needs.
Facility Evaluations
Selecting a new home for a parent is a major decision. An on-site
facility evaluation helps families make the right choice. A
locally-based care manager will evaluate a facility (assisted living,
continuing care retirement community or nursing home) based on the
elder’s need for care. Families receive a comprehensive written report.
