Kristine Chrisopulos named community engagement specialist for Structured Family Caregiving program (SFC) in South Dakota

BOSTON, April 11, 2019 /PRNewswire/ — In partnership with the state of South Dakota, Seniorlink will offer Structured Family Caregiving (SFC) to families caring for a loved one at home. Seniorlink, a tech-enabled health services company, has hired Kristine Chrisopulos to engage and educate the residents of South Dakota about SFC.

Chrisopulos brings experience working with seniors and family caregivers in home care to her new role with Seniorlink. She has focused her career on developing deep expertise in senior care in her home town of Sioux Falls. She most recently served as a Long-Term Services and Supports (LTSS) Specialist in Sioux Falls, where she provided case management support to individuals with disabilities and their caregivers and coordinated in-home support services to keep individuals in their home and surrounded by family.  Prior to her role as an LTSS Specialist, Chrisopulos worked as a Counselor for McCrossan Boys Ranch, where she provided direct care and support services to the youth at the facility.

South Dakota’s SFC program serves people eligible for the HOPE (Home and Community Based Options and Person-Centered Excellence) Waiver, which aims to expand and enhance community-based long-term support services to care for an increasing population of seniors and adults with disabilities. Delivering care to residents in South Dakota’s rural communities is challenging, and a digital coaching solution like Seniorlink’s allows residents to provide care at home instead of in costly, distant institutional settings. Drawing on nearly fifteen years of experience delivering SFC programs in other states, Seniorlink’s SFC program in South Dakota provides a professional coach and a financial stipend to support caregivers who are caring for a loved one at home.
