Seniorlink & Caregiver Homes partners with State Medicaid agencies and their managed-care organizations to deliver a model of care called Structured Family Caregiving (SFC). It provides clinical guidance, educational resources, and financial support, so you can take care of a relative or close friend from the safety and comfort of home.

Caregiver Support from State to State

Medicaid benefits in many States allow family caregivers to receive payment for providing care. But financial support isn’t all you need. You also need real guidance from real care professionals. And that’s where we really help. Because we connect you with an experienced care team — one that’s focused on you and your family’s needs. With us, you’ll always have a partner to lean on. Together, we’ll create a personalized plan that helps you care for your loved one safely at home.

Dedicated Care Team
A team of care professionals will be assigned to work directly with you. Your care team can include nurses, care managers, caregiver coaches, and community health workers. They’ll partner with you to learn about your loved one’s needs, and help create a personalized plan that’s focused on what matters most: helping you deliver the best care, at home.

Scheduled Visits
A member of your care team will visit with you at home or by video conference to check in. They'll answer your questions about caregiving issues and be a resource for support.

Ongoing Guidance & Support
Caregiving is a journey. And we’ll be with you every step of the way. We’ll start with an assessment to better understand your loved one’s needs, as well as your needs as a caregiver. We’ll build a care plan together and connect regularly to offer guidance and support. We’ll work alongside you to stay on the same page, so you can provide the best care possible, each

Digital Tools
As a caregiver in our program, you’ll use a unique online tool called Vela. It’s easy to access, HIPAA secure, and gives you a direct line to your care team. You’ll use it to chat, send messages, and get help — all on your schedule. Vela also makes it easy to manage appointment calendars, set medication reminders, upload files, and safely store important health documents. Which means you can finally keep everything in one place.

Educational Resources
We will make sure you have resources that are specific to your caregiving situation. You will no longer need to spend hours searching for the information you need.

Financial Support
You can receive financial support if you take care of a Medicaid member who is approved to receive our services.

Just remember: Every State Medicaid program is different. The program name, eligibility requirements, and individual benefits vary based on where you live. For instance, the program is called Structured Family Caregiving in some states. While others use: Adult Family Living, Shared Living, or Adult Foster Care. But don’t let this overwhelm you. We’ve got all the information, and we’ll guide you through everything.
